These general terms and conditions apply to all examination and treatment agreements, both oral and written, entered into between the psychologist working at Praktijk Terlouw and the client.

Psychologists work at Praktijk Terlouw according to the professional code for psychologists of the Dutch Institute of Psychologists.

The psychologist has a duty of confidentiality towards third parties and can only provide information to third parties with your permission. The psychologist is only permitted to breach this duty of confidentiality in emergencies. Your consent is not required if consultation with colleagues is necessary, as long as those colleagues are also bound by a duty of confidentiality.

Your information is stored in a secure, digital file. Your file is not accessible to anyone except the practitioner and the client. You have the right to inspect your file but not the right to see the work notes or confidential correspondence of your psychologist. Your file will be automatically destroyed after 20 years, or sooner with your written consent.

At the request of the psychologist, clients must be able to identify themselves by means of a valid identity document. Clients must ensure that Praktijk Terlouw is aware of their correct home address, email address and a telephone number where they can be reached. Changes must be communicated as soon as possible, in writing or by email to anja@praktijkterlouw.nl.

Praktijk Terlouw does not have any contracts with health insurance companies. However, the costs of the treatment are usually fully or partially reimbursed. Ask your health insurance company about this. In order to be eligible for reimbursement from your health insurance company, you need a referral from your general practitioner, stating that the client has been referred for Generalist Basic Mental Healthcare and the suspicion of a mental disorder. The referral should include the name and date of birth of the client, and the name and AGB code of the referrer. The referral must be dated on or before the date of the first appointment and may not be more than six months old. After the end of the treatment you will receive a final invoice for the amount you can pay to Praktijk Terlouw. You will also receive an invoice that you can submit to your health insurance company. The Dutch Healthcare Authority determines the maximum rates for treatment that can be claimed from your insurance company.

If you are unable to attend, you must cancel the appointment at least 24 hours in advance by phone, email or app. Appointments that are missed or that are cancelled too late will not be reimbursed by the insurance company. The rate for this is 50% of the session rate.

Payment for the consultations must be made electronically. This can be done by bank transfer or by PIN payment. You will be asked to pay a deposit, which will be deducted from the final invoice amount. All other amounts claimed for fees and expenses must be paid within fourteen days of the invoice date. The client cannot invoke setting off as a reason for not making payment. Payment must be made on the basis of services rendered and not results achieved. In the event of late payment, the psychologist is entitled to suspend further treatment until the client has fulfilled their payment obligations.

Any liability on the part of Praktijk Terlouw is limited to the invoice value of the services provided. Liability for indirect and/or consequential loss is excluded at all times.

If you have any complaints about the professional conduct of the psychologist treating you, it is recommended that you first discuss these with your psychologist. If you are unable to find a solution, please contact the professional association of which the psychologist in question is a member (NIP, P3NL).
